To be eligible for the NDIS, you must fulfill the following requirements: 

  • have a permanent and significant disability that interferes with your ability to participate in daily activities 
  • be under the age of 65 when you first apply for the scheme;
  •  be an Australian citizen, permanent resident, or New Zealand citizen with a Protected Special Category Visa
  • reside in a region where the NDIS is available.

How to Apply for NDIS? 

Applying Online

If you go to the am I eligible webpage, you will discover an NDIS checklist that you can use to assess your eligibility.

If you fulfill the criteria and want to take part, you may either: call us at 1800 800 110 and ask to make an Access Request; or fill out the form. You can fill out the Access Request Form and email it to NAT@ndis.gov.au or call your state LAC or NDIA office. Call us at 1800 800 110 and kindly ask for an Access-Request.

If you need help filling out the form or making the phone call, contact your Local Area Coordinator, Early Childhood Early Intervention partner, or your local NDIA office. To discover a location near you, go to the contact us page on the NDIS website. 

Access Request questions

As part of the Access-Request process, they will ask you:

  1. to verify your identification and/or the authority of another person to act on your behalf 
  2. questions to determine if you meet the NDIS access requirements of age, residence, and disability. 
  3. questions regarding giving consent to participate in the NDIS and collecting information from other sources

After submitting your Access Request, you may be prompted to supply further information. This could include information on your condition and how it affects your daily life. You can submit copies of available information, such as letters or reports, or you can have your treating physician fill up and sign a form.
